Case Management Application (CMA)

The Case Management Application (CMA) is a unique and valuable public healthcare management system that can significantly supplement the care management process within public healthcare organizations. CMA allows health care professionals to collect key patient medical history, symptoms, clinical and laboratory data. CMA assists healthcare professionals in the facilitation of healthy lifestyles and provide disease risk reduction information among their patients. The patient-specific information is stored in a database configured to provide real-time decision support to caregivers.

CMA allows public health professionals to collect Maternal and Child Health (MCH) Indicators and report Outcome Measures for Perinatal Care. CMA is a comprehensive software solution that is patient centric. This allows patients to participate in several public health programs simultaneously without requiring double/triple entry of patient data and demographics.

The functional pieces are:

Referrals - This function allows various agencies to refer patients into CMA electronically. This function reduces data input requirements for the public health provider and allows faster patient assignment and enrollment into various programs.

Enrollment - This function registers the patient into the program and assigns a case manager and nurse to the patient. This function consists of two maintenance screens, one being patient demographics the other case assignment data. CMA has the facility to search for a patient and calls up the maintenance screen. If the patient is already enrolled, the screen can be used to view and update the demographic information. If the patient is not in the system, the screen is used to add the patient.

Scheduling - The scheduling function keeps track of the next task(s) to be completed, i.e. contact patient for home visit, contact patient by phone, verify doctor's appointments, etc. A schedule table holds information such as; next task to be completed, who is to complete the task, when it is to be started, time frame to complete, and status. The scheduler polls for new tasks that have been posted and distributes the information to the appropriate party. A task table is utilized to store the detail information and the sequencing of tasks.

Data Capture - The data capture function consists of multiple areas depending upon the program being performed. Each data area may have multiple maintenance screens attached to capture patient and procedural information. The Scheduler and the Data Capture functions work together to define the data to be captured next. Once the data has been captured, the task is complete and another task is scheduled. These two functions flip back and forth until the case is closed.

Lab Results Capture - The Lab Results Capture function allows lab results to be transferred electronically into the CMA. This eliminates data input time and greatly reduces inaccuracies. All data is transferred in HL7 compliant format.

Reporting - All the screens have the capability of being printed so that the information can be mailed, faxed, or filed to the appropriate parties. Additionally, consent forms and certificates can also be printed.

Billing - The Billing function allows patient billing.

Programs Currently Covered:

CMA is an Internet based application set using an end-to-end Oracle solution. This solution allows any computer with an Internet Browser, and appropriate security level, to access and/or manipulate information in the database. Additionally, because CMA is a true web based solution it can be deployed in a Wireless Web environment.

Security of CMA is a core feature of the architecture. Security features such as a firewall, public key encryption, secures sockets, role based security and user authorization is implemented. CMA is HIPAA compliant to anticipated rules.

CMA is easily expanded to include additional public healthcare programs. CMA is available as an Internet Hosted Application and can be implemented in a matter of days in the typical public healthcare environment.
